Student/Tutor Agreement (Sample)

The parties to this Agreement are the student ("Student"), his or her parent(s) or legal guardian, the tutor ("Tutor"), and the school representative (Ken Mularski or a requesting teacher or activity leader). The intent of the Agreement is to establish the terms, as set forth below, of the Student/Tutor relationship.

I, ______________(Student), promise that, in return for free periodic (normally weekly) tutoring sessions by the below named Tutor from the Harvard "Adopt-a-School" program, I will:

  1. Report on time for each agreed upon session with the Tutor;
  2. Prepare adequately for each session;
  3. Tell the Tutor at the prior session or otherwise ahead of time what topics it would be helpful to work on at the next session;
  4. Work diligently toward agreed upon goals.

In the event that Student is unable to attend an upcoming session, Student promises to contact Tutor using the number(s) provided as soon as possible, but not later than three hours before the scheduled tutoring session. In the event that Tutor cannot be reached personally, Student agrees to promptly contact Mr. Mularski (in person or @773-534-0034 x 48175).

Should Student fail to contact Tutor or Mr. Mularski as provided above, and Tutor appears for an agreed upon tutoring session which Student does not attend, Student agrees that:

  1. After a first such missed tutoring session, Student will be responsible for calling Tutor the night before each subsequent tutoring session to confirm Student's intention to attend the next day;
  2. After a second missed tutoring session Tutor may decide other consequences, which may include the possibility of discontinuing the tutoring services.

I, ___________________ (Tutor), promise that in return for Student's complying with the above provisions, I will:

  1. Timely attend each agreed upon tutoring session;
  2. Prepare adequately to assist Student with the topics on which Student has requested to work.
  3. Provide feedback to the Overall Tutoring Coordinator (canderson@digitalinnovations.com) to be shared with other tutors and continually improve the program.

In the event that Tutor is unable to attend a tutoring session, Tutor promises to:

  1. Notify Student directly if Tutor is aware by at least the prior evening of Tutor's inability to attend the session;
  2. Notify Mr. Mularski (@773-534-0034 x 48175) if Tutor is unable to contact student or learns of the inability to attend on the day of the tutoring session.
