Friends of Payton 2008 - 2009 Student directory

This contact information will be used for the Student Directory that will be printed by Friends of Payton (FOP) and sent out to all families this fall. Please do not include any information you do not want to see in print.

We prefer that you fill out this online form, but if you prefer, you may submit a paper copy by printing out and filling in this form.

Information should be submitted online or turned into the main office  by Monday, September 8^th.

To complete the online form, please enter your student's ID number.  Their ID number is an 8 digit number that can be found on their Payton ID, schedule, report card, etc.  If your student is an incoming freshman who is new to CPS, their ID number will be printed on the schedule they get at orientation.  If you have more than one student at Payton, choose one ID number.  You will be prompted to enter additional ID numbers later.
